Support Coordination

Community Carer Network fulfils the promise of ensuring participants live a life within their choice and control. Community Carer Network supports participants to access services that support their goals and aspirations in life by liaising with mainstream, community, informal and funded supports. Community Carer Network also commits to the preservation and respect of the rights of people with disability.

Respite Care

Our respite care services are designed to provide temporary care for your family members, giving you the necessary opportunity to rest and relax. As a carer, ensuring your needs are recognised and met can be just as important as the quality of respite care you provide your loved ones. Community Carer Network understands the stresses and rigours of quality long term home care and so offer you a team of qualified respite carers, which give you the time and space to rejuvenate and recharge, returning to your respite care duties fully refreshed.

Personal Care

Personal care supports relate to assistance with daily personal activities including assistance with, or supervision of, personal tasks of daily life.

  • Personal hygiene, including showering, bathing, oral hygiene, dressing and grooming,
  • Toileting, bladder and bowel management and menstrual care.
  • Eating and drinking.
  • Attending appointments.
  • Mobility and transferring such as moving in and out of bed, on or off the toilet.

Social Activities

There are many social activities for disabled people. These activities help them to get more self-confidence, make friends, and also learn new skills. They also encourage the person to do things on their own or in a group with other people.

Medication Assistance

Medication assistance is one of the most important services provided by the Community Carer Network. Without this service, many disabled people would not be able to live independently. Community Carer Network provides home visits for disabled people who need help with medication reminders, administration and dispensing of medicines. This service is often provided by volunteers who are trained in the skills that they need to provide this service.

Transport

The transport service is one of the most important needs for people with disabilities. It is a service that enables them to have access to the community and to be able to live independently from their homes. This service is provided by a community carer network that is in charge of transporting people with disabilities by car or wheelchair. In order for this service to be efficient, it needs to have a set of rules and regulations that define how it should work. These rules and regulations are designed in order for people with disabilities to feel safe when they use this transport service, as well as for the drivers who provide the transportation.

Cooking & Cleaning

The service helps people with disabilities to live independently. By cooking and cleaning for them, they are able to focus on the things they enjoy doing. The service is provided by a community carer network of volunteers who are trained in cooking and cleaning. They come to the house of the person with disability and provide their services for an hour or two every week.

Gardening Service

The Gardening Services were given by community carer networks who were looking for ways to support people with disabilities. Their goal was to provide people with disabilities an opportunity to have their own garden and enjoy the benefits of fresh produce.
